Grace Lezon, NP, is a nurse practitioner with Respiratory and Sleep Specialists who provides pulmonary and post-acute care to patients in skilled nursing facilities throughout the Chicago area. Working in close partnership with the group’s physicians, she manages the ongoing respiratory needs of patients recovering from illness, surgery, or hospitalization in the long-term care and rehabilitation setting.
In the skilled nursing environment, Ms. Lezon cares for patients with conditions such as COPD, pneumonia, and chronic respiratory failure, coordinating closely with patients, families, and facility staff to support recovery and quality of life. She is committed to bringing specialist-level respiratory care directly to patients where they live and recover.
